CONTRACT AGREEMENT

Sunflower Farm Mini Goldendoodles

agree to pay a non refundable deposit of $___________ to hold a

_________________________ until ___/___/___. Deposit is only refundable if

death or illness occurs before pick up date. Breeder is

not expected to hold puppy past the pick up date unless special

arangements have been made. (delay in ground shipping, training,

etc.) Breeder will attempt to contact buyer three times if puppy has not been picked

up by agreement date. After third attempt to contact buyer, breeder is no longer

obligated to hold puppy and contract is broken.

I also understand that the balance of the purchase price (and training if applicable)

is due when the puppy is 5 -6 weeks of age, or when the pick is made (no later than

six weeks of age). This in fairness to others who may be on the waiting list for a

puppy, and are ready to pick up a puppy.

I agree to have the puppy spayed or neutered by 6 months of age. I also agree to send the breeder proof of this surgery from DVM.

I agree to keep my puppy on NuVet canine vitamins for the first full year. Sample and order code have been provided at time of purchase.

I have read and understand the health guarantee and contract agreement for

the puppy that I am purchasing from Sunflower Farm Mini Goldendoodles.

I agree to the terms of said contract.
